He's one of the boss monsters from Silent Hill 2, and the unofficial mascot for the whole series.

Not really, though from clues in the game, he's a monster hallucination (sort of) based off of Civil War executioners from when the town Silent Hill was a prison camp for soldiers. Or something like that.

Silent Hill isn't really big on the whole "exposition" thing.
